Below is a list of common items and information that will be needed during your appointment with the director at the funeral home.
- Clothing (including undergarments)
- Social Security Number
- Date and place of birth
- Full name of both parents (including mother's maiden name)
- Educational background
- Work history
- Marital status (spouse's full name)
- Names of children, grandchildren, great-grandchildren, siblings and spouses
- Veterans discharge papers
- Recent photograph (cosmetic purposes)
- Religious affiliation with clergy name and phone number
- List of pallbearers (if applicable)
- A collection of valued photographs (30-50) for DVD Memorial Tribute
- Payment (life insurance policies, check or credit card)
